GRATEFUL PAWS LLC SERVICE AGREEMENT
Your pet is important to us. We want to assure you that every effort will be made to make your pet’s visit as pleasant as possible. Please read & sign the agreement below to acknowledge our policies and procedures.
Grateful Paws LLC employs several mobile units around the CSRA. Because your grooming professional is traveling to my you on wheels, we will provide you with a time frame an hour prior to an hour after your pre-scheduled appointment time. By signing this agreement, you acknowledge you intend to have someone present within the entirety of your predetermined time frames unless otherwise specified to our scheduler.
I understand it is my responsibility to confirm this information with the scheduler. I acknowledge my appointment may be forfeited in the case of a no-show and I may be subject to any and all fees associated.
Grateful Paws LLC reserves the right to increase pricing when necessary for any variety of factors including but not limited to time allotted, coat condition, and pet temperament and to refuse service if your grooming professional feels uncomfortable proceeding with the groom for any reason, including but not limited to: nervousness, stress, aggression, health, or parasite infestation.
AGGRESSION
I understand I MUST inform Grateful Paws LLC if my pet has any history of aggression to ensure the safety of my grooming professional. Should my pet become a threat to its grooming professional, I understand the groom will be stopped and I may incur any fees for services completed.
PRE-EXISTING CONDITIONS
By signing this agreement, I acknowledge that I have expressly informed Grateful Paws LLC to the best of my knowledge of ANY AND ALL pre-existing conditions for my pet * including matting, sores, or abrasions. * I understand it is in my pets’ best interest to disclose any information related to the health and physical condition of my pet prior to the groom. I understand failure to disclose this information may result in a forfeited appointment and the inability to rebook with Grateful Paws LLC.
SENIOR PETS
While we will do our very best to keep your pet safe and comfortable during the grooming process, grooming can cause stress that may exaggerate or expose underlying issues. By signing this agreement, I acknowledge there are risks associated with grooming services provided to a senior pet. I release Grateful Paws LLC from any and all liability should any emergency occur. I am solely responsible for any and all medical bills related to my animal. Should my grooming professional deem my pet unsafe to groom, I acknowledge the groom will be stopped immediately and I will be charged for the services completed.
EMERGENCIES
Should an emergency arise, and the owner of the pet is not present, I employ Grateful Paws LLC to act as my agent and seek medical treatment for my animal. I agree any and all medical costs will be the owner’s responsibility.
MATTING
Pets with severely matted coats require extra attention from our grooming professionals. Grateful Paws LLC will not cause serious or undue stress to pets by de-matting. If my groomer deems it unreasonable or pain-inducing to dematt my pet, I understand my pet will need to be shaved to proceed with service regardless of my preference. If matting persists with each appointment, I understand I may be required to maintain a routine at my groomers’ discretion to maintain service with Grateful Paws LLC.
CANCELLATION POLICY
I understand it is my responsibility to maintain my appointments. Should my grooming professional arrive within my designated time frame and I am not present, I acknowledge my appointment is subject to forfeit and I may incur 50% of my full grooming price for any missed appointments the day of reflect my grooming professional’s time, gas, and lost income.
Grateful Paws LLC reserves the right to deny future service should I cancel my pets’ grooming appointments. I understand a last-minute cancellation fee of $50 may apply should I fail to cancel my appointment within 24 hours.
LIABILITY
Grateful Paws LLC employs extensive safety and sanitary precautions to ensure your pets’ safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: clipper burn, exposure of the quick, reactions to flea dip treatments and de-shedding treatments, hair splinters, and sharp nails (after clipping).
By signing this agreement, you (the client) agree to relieve Grateful Paws LLC, and its agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Grateful Paws LLC. The pursuit of any veterinary, behavioral, or rehabilitation services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Grateful Paws LLC and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Grateful Paws LLC and its agents.
